Health Navigator

Job overview

The Health Navigator role is onsite full time in San Jose, CA, with pay of $24.04–$25 per hour. Roots Community Health Center serves individuals and families who need health and social services. The navigator builds trust with assigned participants, assesses barriers, connects them with public benefits and services, and provides health coaching. The role also supports clinic staff and providers, completes required documentation, and helps participants access care and resources.

What you'll do

• Build rapport with participants
• conduct face-to-face visits and outreach calls
• assess psychosocial, health, housing, food, legal, and other barriers
• assist with enrollment in public benefit programs and medical and social services
• provide health coaching
• support clinic operations
• complete required documentation and reports
• help maintain a clean and safe site.
