What you'll do

• Provide social work services, support, and advocacy to residents
• maintain information on community and health resources
• communicate with families about health changes and discharge planning
• participate in resident rights and care planning
• coordinate transfers to appropriate medical care
• arrange adaptive equipment and personal items
• assist residents and families with healthcare decisions
• provide individual counseling
• and communicate with the interdisciplinary team about resident needs and admissions and discharges.
