Care Coordinator

Job overview

The Care Coordinator role is hybrid at STRIDE Community Health Center’s Wheat Ridge Clinic in Wheat Ridge, CO, with three onsite days and two remote days per week; full-time onsite attendance is required during training. The schedule is Monday–Friday, 7 a.m.–5:30 p.m. Pay is $20.67–$24.03 per hour. STRIDE provides comprehensive healthcare services to individuals and families in the Denver Metro area. The coordinator supports quality and value-based care by connecting patients with internal and external services, managing referrals, and providing basic health education. The role helps identify social and health-related barriers, connect patients with appropriate resources, and follow each referral through completion.

What you'll do

• Coordinate internal and external patient services
• use evidence-based screening tools to document social determinants of health and health-related social needs
• provide resources and referrals
• maintain community resource information
• document and communicate closed-loop follow-up
• provide basic health education
• manage referrals and partner records
• conduct targeted outreach and scheduling
• support quality improvement activities.
