What you'll do

• Engage with individuals considering behavior change or treatment onsite and in the community
• provide outreach, advocacy, mentoring, and recovery support
• help patients attend treatment and navigate healthcare options
• raise awareness of social support services
• provide non-clinical crisis support
• develop recovery plans with patients
• complete required documentation.
