Outpatient Program Care Coordinator

Job overview

The Outpatient Program Care Coordinator role is hybrid in Oakland, CA, with in-person client services and remote documentation. The hourly pay range is $34.62–$36.00, with an additional 10% Spanish-language differential for qualified candidates. WestCoast Children’s Clinic provides mental health services to Bay Area children, youth, and families through a client-centered, trauma-informed, collaborative approach. The coordinator connects screened referrals with outpatient care teams and supports waitlisted youth and families while they await assignment. By coordinating scheduling, follow-up, case management, and communication with providers, the role helps clients access appropriate care and supports team-based services.

What you'll do

• Track youth referrals in electronic health records, spreadsheets, and Google Workspace
• provide short-term case management and care coordination for waitlisted youth
• complete risk assessments and provide immediate crisis resources
• obtain clinical documentation
• organize program orientations
• conduct outreach to re-engage clients
• serve as backup Child & Family Team meeting facilitator
• review care plans for regulatory compliance
• complete Medi-Cal documentation within 48 hours
• monitor client feedback
• strengthen community partnerships and plan monthly gatherings.
