Hospice Registered Nurse (RN) Case Manager

The Hospice Registered Nurse (RN) Case Manager role is onsite in Milwaukee, WI, with compensation of $40 per hour. By Your Side Hospice Care supports patients and families across southeastern Wisconsin with end-of-life care focused on dignity and respect. The role coordinates individualized hospice care and provides direct nursing support so patients’ needs and preferences guide care decisions. It contributes to patient comfort and quality of life through assessment, symptom management, family education, and collaboration with the interdisciplinary team.

What you'll do

• Conduct comprehensive patient assessments and develop individualized care plans
• coordinate care in accordance with hospice policies
• provide nursing care, symptom management, and pain control
• educate patients and families about end-of-life care and resources
• collaborate with interdisciplinary teams
• document progress and maintain medical records
• participate in on-call rotations
• advocate for patient needs and preferences.
