The Social Worker – Nursing and Rehabilitation role is onsite in Ithaca, NY, with pay of $20–$22 per hour. Cayuga Nursing & Rehabilitation Center is recruiting a full-time social worker to provide social services for residents. The social worker supports residents’ social and personal care needs, serves as a liaison among medical staff, families, and outside agencies, and helps coordinate admissions, discharges, and care-plan meetings. The role also gathers social histories and maintains information about community health and welfare resources.

What you'll do

• Provide social services to residents
