Compensation is $54–$76 per year. The Home Health Speech Language Pathologist (Per Diem) role is an onsite position based in Eastland, TX. 00 - $76.00. This role supports the CenterWell Home Health team in delivering personalized rehabilitation care to patients in their homes. The role exists to help patients regain strength, mobility, and independence by assessing and treating disorders of speech, voice, language, hearing, and swallowing. The pathologist develops and implements treatment plans, provides patient and family education, and coordinates care with physicians to ensure optimal recovery outcomes.

• Evaluate and provide speech/language pathology services
• develop and review Plans of Care
• administer diagnostic tests
• provide patient/family counseling
• maintain clinical documentation
• participate in discharge planning.
