Temporary Case Manager, Stability

The Temporary Case Manager, Stability role is onsite in San Francisco, CA, with home-based participant support and local and long-distance travel. Hamilton Families works to end family homelessness in the San Francisco Bay Area; this role is part of the Housing Solutions team. The case manager supports families in a rental subsidy program through housing goal-setting, advocacy, resource connections, documentation, and follow-up. The work helps participants build self-sufficiency and maintain stable housing while coordinating with internal teams and community partners.

What you'll do

• Provide home-based support to around 20 families
• help set housing goals and connect participants to budgeting, employment, legal, and other resources
• assess needs and connect families to services
• manage documentation and electronic case records
• check in with program graduates
• maintain confidentiality
• collaborate with partners
• help organize community events
• represent the organization at community meetings.
