The Registered Dietitian / Nutritionist role is onsite in Raleigh, NC; compensation is not specified. Amplify Health provides integrated community health services and serves patients across Wake and Franklin counties. The role exists to provide nutrition care and education that supports disease prevention and improved health outcomes. The dietitian assesses patients, develops individualized nutrition plans, collaborates with care teams, and contributes to chronic disease programs and community outreach.

What you'll do

• Provide medical nutrition therapy and individualized counseling
• conduct nutrition assessments and develop care plans
• support patients with diabetes, hypertension, and obesity
• track progress and adjust plans
• collaborate with primary care providers and care teams
• participate in case reviews and population health initiatives
• lead nutrition education sessions and group classes
• support community outreach and health promotion.
