Health Homes Care Manager

What you'll do

• Coordinate comprehensive care management, case planning, and crisis response
• maintain relationships with DSS, schools, community agencies, and service providers
• complete accurate reports and documentation
• develop therapeutic relationships with families
• assess strengths and needs
• support intervention planning, skill development, and independent living
• monitor and improve clinical programming.
