Case Manager

What you'll do

• Conduct client intake interviews
• help clients identify and implement goals
• hold regular case-management meetings
• facilitate life-skills and other groups
• maintain client charts and confidentiality
• participate in multidisciplinary staffing
• explain program rules and expectations
• assist with events and presentations
• report recommendations and program statistics to the Director of Operations
• prepare weekly, monthly, and quarterly reports
• recommend program improvements.
