Job overview

The Case Manager works onsite across in-home, office, and community settings in Maricopa, AZ. Compensation is not specified. EMPACT–Suicide Prevention Center provides behavioral health counseling, crisis intervention, and suicide prevention services. The case manager coordinates behavioral health services for adults, children, and families, helping clients access support and maintain stability in the community. The role works with families, natural supports, and community partners while documenting services and coordinating care according to Arizona’s practice model.

What you'll do

  • Provide direct services, crisis intervention, transportation, education, client stabilization, supervision, and behavior support
  • facilitate and follow up on Child and Family Team or Adult Recovery Team meetings
  • document meetings and services
  • engage natural supports
  • develop relationships with agency stakeholders
  • make referrals
  • maintain client records and compliant clinical documentation
  • attend weekly supervision.

Why this role

Requires a bachelor’s degree and one year of behavioral health experience, an associate’s degree and two years of experience, or a high school diploma and four years of experience. Spanish is preferred; a Spanish language differential is available.
