What you'll do

• Document case management plans and ongoing activities
• review precertification requests for medical necessity
• monitor clinical and non-clinical staff activities
• identify insurance coverage or other payment sources
• identify risk factors and barriers to care
• connect patients with services and resources
• communicate patient preferences and advocate for their needs
• screen for healthcare needs
• develop patient-focused plans
• educate patients, families, and caregivers about case management
• float among clinical services when qualified and competent.
