Patient Care Coordinator

What you'll do

• Coordinate care for newly admitted infusion or enteral therapy patients
• communicate with pharmacists about drug delivery methods and medication needs
• conduct resupply calls and clinical screening
• follow payer and regulatory requirements
• enter orders and maintain patient records
• obtain prior authorizations and verify coverage
• identify nursing needs and coordinate home health or nursing agency services
• address irregular usage and shipping or service issues
• collaborate with referral, clinical, and inventory teams
• participate in quality assurance and process improvement and complete billing documentation.
