Part-Time Client Care Manager

What you'll do

• Conduct in-home client assessments and reassessments
• develop, review, and update Plans of Care
• conduct supervisory and quality-assurance visits
• follow up with clients and families after new starts of care
• monitor satisfaction and identify concerns
• report changes in client condition or needs
• assist with caregiver-client matching
• coordinate with the Scheduler/Care Coordinator
• coach caregivers
• address client and family concerns
• support new starts of care
• document assessments, visits, incidents, and follow-ups
• escalate significant concerns
• maintain confidentiality and professional boundaries.
