Lead Care Manager, Enhanced Care Management

The Lead Care Manager, Enhanced Care Management role is onsite in Los Angeles, CA, with compensation of $28–$30 per hour. The position supports Correctional Health Treatment Centers’ CalAIM Enhanced Care Management program, which coordinates clinical and non-clinical services for people with complex needs. The role serves as a primary contact for clients and their support networks, assesses needs, develops and updates care plans, and coordinates services with community providers and multidisciplinary teams. It helps clients navigate health and social service systems and supports progress toward individualized health and wellness goals.

What you'll do

• Serve as a primary contact for program clients, families, authorized representatives, caregivers, and multidisciplinary care teams. Conduct assessments and develop individualized care plans
• coordinate services with internal and community providers
• support SMART goals and revise plans as needed
• maintain communication with agencies and care teams
• prepare progress reports and case notes
• develop transition plans
• provide member and family education
• track assessments and screenings
• and maintain client files.
