What you'll do

• Lead dementia care program design and implementation
• supervise and mentor nursing staff, care coordinators, and support teams
• oversee individualized care plans and documentation
• ensure compliance with long-term care regulations, HIPAA, and Medicare guidelines
• review medical records, MDS, and clinical notes
• train staff
• collaborate with interdisciplinary teams on outcomes, shift management, and resource allocation.
