Registered Nurse (RN) Case Manager, Home Health

The Registered Nurse (RN) Case Manager role is onsite in Browning, Montana, with a self-directed schedule and home-health visits in the Butte market and surrounding area. What you'll do

• Provide in-home care to patients with illnesses, injuries, and chronic diseases
• conduct assessments and admissions and develop care plans
• work with physicians and oversee LPNs, CNAs, and aides
• educate patients and families about care plans, medication, therapy, diet, exercise, and home safety
• oversee clinical staff in the Butte market and surrounding area.
