What you'll do

• Conduct home, school and community visits and help families access resources
• supervise visits when needed and report relevant observations
• provide transportation support for children and families when the case manager is unavailable
• upload and document activities in the electronic case management system
• monitor visitation schedules
• report safety needs
• provide homemaker training
• participate in family team conferences and attend court hearings as needed
• provide emergency on-call services on a required rotational basis.
