What you'll do

• Guide patients through sleep-disorder diagnosis and treatment
• coordinate home sleep apnea testing and in-center sleep studies
• educate patients on sleep health and prescribed therapies
• monitor compliance and treatment effectiveness
• follow up on physician orders and care plans
• assist with portal enrollment
• coordinate care transitions after hospital discharge
• maintain EMR documentation and track outcomes and satisfaction.
