What you'll do

• Enter referrals accurately and promptly
• receive referrals and request supporting documentation
• document patient information in the electronic health record
• send referrals to a care coordinator or covering staff within 15 minutes
• ensure consents and records meet organizational and regulatory standards
• coordinate with admission nurses and hospital nurse care coordinators
• maintain referral and admission logs
• document referrals not accepted and follow-up plans
• enter daily updates and admissions
• assist with home health and palliative referrals as needed.
