Case Manager, Targeted and Crisis Case Management

The Case Manager, Targeted and Crisis Case Management role is an onsite, full-time position in Milwaukee, WI, with work across community sites and clients’ homes. Compensation is not specified. St. Charles Youth & Family Services provides case management and rehabilitative support to people living with mental illness and/or substance abuse who need help maintaining stability in the community. The case manager coordinates ongoing client support, crisis and safety planning, and treatment resources. The role contributes to client stability through documentation, collaboration, community representation, and crisis response, including an assigned rotating after-hours crisis phone.

What you'll do

• Provide case management throughout client enrollment
• develop crisis and safety plans
• coordinate agency and community resources and treatment planning
• document provider notes and updated plans
• collaborate with clients and team members
• represent the agency at court, school, medical, and community settings
• assist with crisis intervention and response
• participate in rotating after-hours crisis phone coverage.
