Consumer Direct Services Specialist

Through participant contacts, home visits, field services, documentation, and customer support, the role helps maintain program compliance and connect participants with appropriate assistance.

What you'll do

• Complete monthly participant contacts and annual home visits
• document service monitoring and required paperwork
• conduct face-to-face intakes and provide participant and attendant training
• conduct spot-check or unannounced home visits as directed
• track and review participant records
• report suspected abuse, neglect, exploitation, or program fraud according to policy
• provide customer support and maintain case notes.
