What you'll do

• Assess patients and prioritize care needs
• develop and update individualized care plans and SMART goals
• coordinate transitions after hospitalization or skilled nursing care
• review discharge information and arrange follow-up
• conduct follow-up calls and medication reconciliation
• provide disease-management education
• connect patients with medical, social, and community resources
• document care and outcomes in the EHR.
