The Registered Nurse (RN) Case Manager Lead is an onsite role based in Greensboro, NC. Compensation is not specified for this position, which supports the hospital's complex case management team. - Identify patients across the hospital with extended Lengths of Stay.
- Develop and implement comprehensive, patient-centered discharge plans.
- Co-Lead and facilitate inpatient interdisciplinary Complex Case Rounds.
- Engage with known complex case patients in the ED.
- Collaborate with ambulatory and community-based teams.
- Monitor avoidable days and delays.
