What you'll do

• Coordinate and provide individual, group, and family counseling and biopsychosocial assessments
• use agency assessment tools
• identify client needs
• prepare clinical summaries and person-centered care plans
• document progress and treatment
• prepare transition notes
• participate in crisis intervention, utilization reviews, and weekly treatment team meetings
• provide referrals and appropriate interventions
• follow professional ethics and safety procedures.
