What you'll do

  • Provide one-on-one independent living skills training in home and community settings
  • develop and implement individualized service plans
  • teach meal preparation, budgeting, personal hygiene, and household management
  • support access to community resources, transportation, and social services
  • monitor and document progress
  • update care plans as needed
  • collaborate with case managers, families, and support teams
  • encourage self-advocacy and community integration.
