Hospice Registered Nurse (RN) Case Manager

What you'll do

• Manage a hospice patient caseload
• complete admission, routine, and recertification visits and assessments
• develop care plans with the interdisciplinary team
• provide pain and symptom management
• educate patients and families
• coordinate with aides, social workers, chaplains, and volunteers
• document visits
• participate in weekly team meetings
• take part in the shared on-call rotation
• support families through end-of-life care and mentor hospice aides.
