Service Coordinator

Job overview

The Service Coordinator role is in-office in Duluth, MN. Starting pay is $21 per hour, with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m. ConnectAbility of MN coordinates authorized services for individuals and families in the community. The coordinator manages assigned cases or shared service queues, interprets approved Service Agreements, and tracks service delivery and documentation. By coordinating clients, case managers, and vendors in line with Minnesota DHS waiver requirements, the role supports respectful, consistent, and compliant services.

What you'll do

• Coordinate services authorized through Service Agreements
• interpret agreements to start, stop, adjust, or hold services under DHS waiver rules
• monitor service status and thresholds
• communicate changes to case managers, clients, and vendors
• support resolution of denied claims and accurate billing
• maintain client records and service information in Salesforce
• run routine reports
• obtain vendor quotes and coordinate services within authorized budgets
• communicate schedules and changes
• onboard vendors as needed.
