What you'll do

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required

• monitor care-plan effectiveness and document progress
• coordinate behavioral health, LTSS, and community resources
• assess medical necessity and authorize appropriate waiver services
• evaluate covered benefits
• facilitate interdisciplinary care team meetings
• use motivational interviewing
• address barriers to care
• identify critical incidents and develop prevention plans
• travel locally an estimated 25–40% as required.
