What you'll do

• Partner with members and families to identify supports and direct services
• collaborate with clinical and social support colleagues
• conduct face-to-face visits and assess physical, behavioral, functional, psychosocial, financial, employment, and independent-living needs
• apply criteria to authorize home- and community-based services and confirm services are provided. Home visits and travel to members’ homes are part of the role.
