What you'll do

• Assess patient needs and develop, implement, update, and evaluate individualized care plans in accordance with physician orders
• educate patients and families

• communicate changes in patient status
• connect families with social work and community resources
• supervise LPNs and HHAs
• participate in patient and team conferences
• schedule visits and manage caseload
• precept newly hired nursing staff.
