NorthKey provides mental health, substance use, and developmental-disability services in Northern Kentucky. The nurse supports outpatient behavioral-health clients and works with physicians and nurse practitioners on medication management, assessments, and care planning. The role also supports screenings, documentation, medication processes, and interdisciplinary coordination to promote continuity and quality of care.
What you'll do
- Assist physicians and nurse practitioners with medication management and service plans based on assessment data
- support hospital discharge clinics and medication assessments
- conduct routine screenings for additional therapeutic and physical-health services
- consult with staff
- document service plans, progress notes, medication-informed consent, and treatment-plan reviews
- ensure patients are seen routinely before medication refills
- monitor caseloads for changes in mental status or medical condition
- handle medication orders according to policies and regulations
- collaborate with staff and external professionals.
