Care Management Extender

Job overview

The Care Management Extender role is onsite in Lumberton, NC. Compensation is $17.36–$22.29 per hour. The role supports True Team Medical’s Tailored Care Management team, which coordinates integrated behavioral and medical care. This position helps members with complex needs connect to care and community resources, supports individualized service planning, and promotes communication and continuity among members, families, and providers.

What you'll do

• Conduct outreach by phone and in person
• monitor members for clinical changes and treatment complications
• assist members and families with individualized service plans
• coordinate whole-person care with primary care and other providers
• connect members to internal and community resources and follow up on referrals
• help ensure treating providers have current test results and information.
