Case Manager

Job overview

The Case Manager role is onsite at an inpatient detox and residential treatment facility in Fort Collins, CO. Compensation is $21–$25 per hour. Avenues Recovery operates drug and alcohol rehabilitation centers offering detox, residential, partial hospitalization, intensive outpatient, and outpatient services.

What you'll do

• Advocate for clients and address external barriers to treatment
• initiate case management at intake
• monitor client insurance coverage during treatment
• provide ongoing case management
• arrange housing, financial aid, legal aid, and vocational training as needed after discharge
• coordinate discharge planning for continuing care, housing, and sober support.
