Registered Nurse (RN) — Home Care Supervisor

What you'll do

• Meet new patients, explain services, and complete start-of-care assessments
• develop individualized care plans
• monitor progress, adjust plans, and report condition changes
• teach or supervise household members assisting with care
• instruct and oversee home-care staff
• review caregiver documentation and conduct supervisory visits
• lead or oversee staff training
• coordinate clinical notes and compliance records
• participate in interdisciplinary meetings and quality reviews
• maintain community and referral relationships
• travel regularly within the assigned area.
