What you'll do

• Manage and collaborate on patient care plans
• provide care to patients in Western Pennsylvania and surrounding communities
• complete charting and documentation by required deadlines
• manage daily workflow and schedule
• maintain clinical quality and EMR documentation standards. Local travel and driving to patient homes are required
• the posting estimates driving at approximately 20% of the day. On-call coverage is Friday, Saturday, and Sunday from 5 PM to 10 PM
• the schedule includes 8 units Saturday and Sunday and two holidays per year.

Why this role

Pay is per visit; the posting does not state a numeric rate.
