Travel Social Worker

Job overview

The Travel Social Worker role is onsite at a rural hospital in a remote town several hundred miles northwest of Anchorage, AK. Pay is $60–$65 per hour. The hospital serves patients in its Acute Care Unit and Primary Care Clinic. The social worker provides medical social work and case-management support to patients of all ages. The role helps coordinate discharge planning, placements, crisis intervention, and care coordination during a six-month assignment.

What you'll do

• Provide medical social work services to patients of all ages in the Acute Care Unit and Primary Care Clinic
• respond to requests for case management, discharge planning, nursing home and assisted-living placements, risk management, crisis intervention, and care coordination
• coordinate nursing home and assisted-living placements.
