What you'll do

• Lead the clinical care team, including advanced practitioners, medical assistants, care managers, and community health workers
• care for a panel of senior patients
• coach providers on panel management, documentation, and indirect care time
• partner with the Care Center Manager on patient experience, STARS/HEDIS, care coordination, utilization, and cost of care
• champion talent, culture, clinical innovation, and compliance
• represent Hopscotch in community education, engagement, and events.

Why this role
