What you'll do

• Execute physician-prescribed care plans
• document care
• administer medications, treatments, and therapies
• assess patients and coordinate care
• monitor vital signs, intake and output, skin integrity, and condition changes
• provide wound care
• communicate with families, physicians, and case managers
• educate patients, families, and home health aides
• collaborate with and supervise nursing assistants and personal care attendants.
