Home Health Aide

Job overview

The Home Health Aide role is an onsite, part-time position in Middleburg, FL, paying $15–$18 per hour. Heart2Heart HHC provides one-on-one care in patients’ homes. The role exists to follow individualized care plans and provide practical and personal assistance that supports patients’ daily needs, safety, and wellbeing. The aide also records and reports changes in patient condition and builds supportive relationships through conversation and attentive care.

What you'll do

• Assist patients with moving in and out of beds, baths, wheelchairs, or automobiles
• change bed linens, do laundry, clean the home, and assist with personal care
• maintain records of care, condition, progress, or problems and report them
• converse with patients to support mental wellbeing and alertness
• oversee medication management and assist as needed.

Why this role
