What you'll do

• Assess participant needs and preferences

• ensure behavioral and crisis plans align with the individual support plan
• report abuse, neglect, or exploitation
• coordinate physical, behavioral, and support services
• research alternatives
• maintain confidentiality
• attend required training and recertification.

Why this role
