What you'll do

• Interview prospective clients in person and by phone
• assess needs and make service referrals
• support transitions from the designated office
• coordinate with Roots staff, correctional facility personnel, and community partners
• complete HMIS and CES training and assessments
• complete homelessness and disability verification forms
• follow up on missed appointments
• maintain records in databases and EHR
• use de-escalation techniques when necessary.
