Per Diem Physician Assistant, Mobile Wound Care

The Per Diem Physician Assistant, Mobile Wound Care role is based in Sunnyvale, CA, with visits in patients’ homes and other community settings across the assigned service area. Compensation is $100 per completed patient visit and $100 per hour for required training, meetings, and designated non-visit work. The role supports Dermacore Wound Care’s mobile care team. The physician assistant assesses and treats wounds, documents care, and coordinates with patients, caregivers, referring agencies, and the interdisciplinary team. Monitoring progress and escalating concerns supports continuity of care and the company’s goal of delivering patient-centered wound care in the community.

What you'll do

• Perform patient and wound assessments and develop individualized treatment plans
• provide wound care visits and procedures within scope and privileges
• manage wound-related recommendations and orders
• monitor wound progression and adjust plans when indicated
• complete same-day clinical documentation
• communicate with patients, caregivers, agencies, physicians, and care team members
• collaborate with support teams
• escalate clinical concerns
• participate in required education, quality initiatives, and meetings
• maintain HIPAA and regulatory compliance.
