CalAIM Community Health Worker

Job overview

The CalAIM Community Health Worker role is hybrid and field-based in Eureka, CA, with approximately 65% of work in the field and 35% remote. Compensation is $21–$24 per hour. Pacific Health Group’s CalAIM programs coordinate community-based services and address social determinants of health to improve outcomes. The worker connects members with healthcare and community resources through outreach, assessments, education, care navigation, and follow-up. The role supports member stability and continuity of services while documenting work and coordinating with interdisciplinary teams.

What you'll do

• Conduct outreach, assessments, and member engagement
• develop Plans of Care
• support care navigation, appointments, transitions, and follow-up
• provide health education and coaching
• coordinate community events and activities
• facilitate group sessions
• assist with public assistance enrollment and advocacy
• document services and reporting
• collaborate with RNs, licensed providers, and interdisciplinary teams.
