Life Skills Coach

Job overview

What you'll do

• Travel to clients’ homes and transport clients during visits
• implement care plans through home and community activities
• track SMART goals and progress through weekly documentation
• communicate with clients, families, and health professionals
• support organization, planning, cognitive awareness, appointments, and household management
• work independently and manage time.
