What you'll do

• Provide personal care and assist with activities of daily living according to the plan of care
• maintain a safe environment
• assist with ambulation and transfers
• assist with exercises as directed
• prepare nutritional snacks as appropriate
• assist with specified self-administered medications in home care under nurse supervision
• perform simple wound, skin, and bowel care as directed where appropriate
• report changes in patient status
• document care and observations
• attend care conferences, meetings, and in-services
• participate in the scheduled on-call rotation for Home Care only.
