Case Manager II – Medicare & Waivers

Job overview

The Case Manager II – Medicare & Waivers role is onsite in Minneapolis, MN, with occasional remote work or schedule adjustments possible as operational needs allow. Compensation is $26 per hour. The role supports Minneapolis Public Housing Authority residents through Agate’s Specialized Housing Stability program. Agate’s mission is to end homelessness and relieve hunger through action and advocacy. This position helps residents and families access and maintain health coverage, waiver services, long-term supports, and other benefits that promote independence, aging in place, and housing stability. It provides resident-centered case management and coordinates with families, county staff, health plans, providers, and community partners.

What you'll do

• Provide individualized case management and benefits navigation focused on Medicare, Medical Assistance, waiver services, disability supports, and long-term services. Assist with applications, renewals, verifications, MnCHOICES referrals and assessments, and follow-up. Coordinate with county staff, health plans, providers, and community organizations
