What you'll do

• Support pre-admission, admission, discharge, and follow-up activities
• conduct facility tours
• coordinate with the liaison on patient acceptance
• track referrals and notify departments of anticipated admissions
• assist with admission arrangements
• obtain payor prior approvals when authorized
• ensure level-of-care approval or required screening when indicated
• complete and distribute admission documentation
• verify Medicaid eligibility and estimated co-pay when appropriate
• track acute hospital admissions
• coordinate weekend and holiday admissions with liaisons
• maintain resident, waiting, and inquiry lists
• coordinate clinical assessment and screening when the liaison is absent.
